THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
SSC Legal Nurse Consultants Group ("SSC") is a medical-legal consulting firm that may receive, access, or work with protected health information (PHI) in the course of providing consulting services to law firms, insurance agencies, healthcare organizations, and related clients. This Notice describes our privacy practices regarding PHI in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations.

This Notice applies to SSC Legal Nurse Consultants Group and all personnel acting on its behalf. It governs our handling of protected health information (PHI) that we receive, access, or create in connection with our consulting services. PHI includes individually identifiable health information in any form — paper, electronic, or verbal.
SSC operates in the capacity of a Business Associate to covered entities under HIPAA. As such, our use and disclosure of PHI is governed by applicable Business Associate Agreements and HIPAA regulations.
2. How We May Use and Disclose Protected Health Information
2.1 Uses and Disclosures Permitted Without Authorization
In connection with our consulting services, SSC may use and disclose PHI for the following purposes without individual authorization:
-
Treatment, Payment, and Healthcare Operations: As permitted under HIPAA, we may use or disclose PHI to the extent necessary to perform our contracted consulting services, which may support treatment coordination, payment processing, or healthcare operations of the covered entities we serve.
-
Required by Law: We may disclose PHI when required by federal, state, or local law, including in response to court orders, subpoenas, or government investigations.
-
Public Health and Safety: We may disclose PHI as required to public health authorities or to prevent or control disease, injury, or disability, as permitted by HIPAA.
-
Judicial and Administrative Proceedings: PHI may be disclosed in response to lawfully issued subpoenas or court orders, consistent with applicable HIPAA provisions.
-
Law Enforcement: We may disclose PHI to law enforcement officials in limited circumstances as permitted by HIPAA.
-
Business Associates: We may share PHI with subcontractors or vendors only when covered by a Business Associate Agreement requiring equivalent privacy protections.
2.2 Uses and Disclosures Requiring Authorization
All other uses and disclosures of PHI — including those not described in this Notice — require written authorization from the individual whose PHI is involved, or from their authorized representative, unless otherwise permitted by law. You may revoke a previously provided authorization at any time in writing.
3. Our Obligations
SSC is required by law to:
-
Maintain the privacy of your protected health information.
-
Provide you with notice of our legal duties and privacy practices with respect to PHI.
-
Notify affected individuals in the event of a breach of unsecured PHI, as required by HIPAA.
-
Abide by the terms of the current version of this Notice.
SSC will not use or disclose PHI in ways that are not permitted or required by law, and will not retaliate against any individual for exercising their rights under HIPAA.
4. Individual Rights Regarding PHI
To the extent that SSC holds PHI about an identifiable individual, that individual may have the following rights:
-
Right to Access: You may request access to PHI that SSC holds about you, subject to applicable exceptions.
-
Right to Amendment: You may request amendment of PHI that you believe is inaccurate or incomplete.
-
Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
-
Right to Request Restrictions: You may request restrictions on how we use or disclose your PHI, though we are not always required to agree.
-
Right to Confidential Communications: You may request that we communicate with you about PHI in a specific manner or at a specific location.
-
Right to a Copy of This Notice: You may request a paper copy of this Notice at any time.
To exercise any of these rights, please contact us using the information below. We will respond within the timeframes required by applicable law.
5. Data Security and Safeguards
SSC employs industry-standard administrative, technical, and physical safeguards to protect PHI from unauthorized access, use, or disclosure. These include:

In the event of a breach of unsecured PHI, SSC will notify affected parties as required by the HIPAA Breach Notification Rule (45 CFR Parts 160 and 164). Notification will occur without unreasonable delay and in no case later than 60 days following discovery of the breach, unless law enforcement requests a delay.
